Dave’s Steroid Use and His Story

In Dave’s story, he tells us he had been on steroids a long time before he had any development of male breasts. He did not notice any changes in his breasts until he stopped the steroids. He tells Dr. Delgado that he did not take any anti-estrogen as he was unaware of steroid side effects.

After he weaned himself off steroids and losing some weight, he became aware of his breast development. He was hoping that if he lost weight his male breasts would also reduce but found that was not the case, he learned that only the fat around the breast tissue would increase or decrease but not the breast tissue. The breast tissue is permanent, and surgery is the only way to get rid of it.

You may know some of the common causes of gynecomastia such as; puberty, steroids, medications, and aging. However, there are some lesser known ones that you should be aware of.

Men have more androgen hormones and women more estrogen, but when the ratio gets out of balance for men, the result could be man boobs.

The following are some more obscure reasons that men may develop male breasts, but Dr. Miguel Delgado, M.D. wants to remind men that sometimes the source of gynecomastia remains unknown.

• Deprivation Diets

If you should go on a strict diet that deprives your body of needed nutrients, testosterone levels will fall, but not estrogen levels. Men may not grow breasts while on a deprivation diet, but they may find when they return to a normal diet, they may develop breasts. The term for this is “re-feeding gynecomastia.” This phenomenon was first noted when prisoners of war returned to the U.S. after World War II. When the soldiers returned to a normal diet, they developed breasts.

• Weight Gain

The ratio of muscle to fat gets thrown off when one gains excessive weight. As the proportion of breast tissue to fat rises, the body will produce more estrogen, the female hormone, and not as much androgen. When this occurs, breasts may develop.

• Alcohol and Beer

Consuming alcohol and beer will put on pounds and if in excess could lead to cirrhosis of the liver.  It is hard for the body to rid itself of estrogen and phytoestrogen which are present in hops that are in beer and other plants in hard liquor. The liver metabolizes hormones, and if the liver is somewhat compromised, it may convert androgen into estrogen leading to female type breasts.

• Cigarette Smoking

It is known that cancer produces hormones for its own growth which results in imbalances and can then grow breasts on men. Various types of cancer may cause gynecomastia such as testicular cancer and lung cancer. More reasons to try to quit smoking for good!

• Rifle Shooting

This may be the most surprising reason for the growth of male breast tissue. It was discovered that 70% or more German military men who perform precision drills, develop gynecomastia. It is unknown what causes a rise in estrogen from a rifle backfire!

Change Surgeons?

This is a good question and needs thoughtful consideration. The answer should depend on what type of revision is needed and why. Highly trained Board Certified Plastic Surgeons that specialize in gynecomastia surgery will explain risks and complications, prior to surgery.

There are Risks

All surgeries come with some risks; the main ones are, infection, bleeding, blood clots, and adverse events related to anesthesia. Although rare, patients need to be aware of the possibility. Additionally and specifically to gynecomastia surgery would be where the original surgeon tried to correct gynecomastia by liposuction alone, without removing breast tissue by surgical excision. Almost all gynecomastia cases need both liposuction and surgical excision; the only exception would be for men that have pseudo-gynecomastia (only fat, and no breast tissue). The second main reason for revision would be; under or over resection of the breast tissue. This would require re-excision for under resection or fat injections or fat flaps for over resection. The American Society of Plastic Surgeons (ASPS) publishes a report each year showing the plastic surgery trends for cosmetic and reconstructive surgery and minimally invasive procedures. The report usually comes out around the end of February or the beginning of March.

In March of 2014 we published the statistics for 2013 that revealed an increase of 11% for gynecomastia surgery, with 22,939 cases performed.

Gynecomastia Surgery Center of Excellence

Gynecomastia surgery is not “one size fits all”, each patient is individual and surgery is specific for that patient. However, the surgery process is fairly standard with additional surgery time allowed as needed.

On the day of surgery, you are to arrive 2 hours prior to the start of surgery. You will be greeted by the nurse who will escort you to the “Gynecomastia Surgery Center of Excellence”. You will be assigned a locker where you may store your clothing; all valuables are to be left at home or with the adult who accompanies you.

Pre-operative Preparations

You are then taken to a pre-operative exam room where the nurse will review your post-operative instructions. Dr. Delgado will meet with you to see if you have any further questions and at this time he will be making surgical markings to guide him in surgery.

At this point, patients remember very little as the general anesthesia kicks in, and you go into a very comfortable sleep. Once in the recovery room, you will be attended by our recovery nurse who will be monitoring your vital signs and making sure you are kept comfortable. The use of Steroids among bodybuilders and men who compete in professional sports is not unusual. Steroids are known to increase muscle growth for the bodybuilder and performance can be enhanced for the athlete.

Up until 1990, steroid use was legal, and then Congress added steroids to Schedule 3, which puts it into the same category as controlled substances such as; heroin, cocaine, and LSD. Unless a doctor prescribes anabolic steroids for a medical condition, they are illegal to use.

Dr. Delgado warns his male breast reduction patients about using steroids and purchasing them online or by mail order. They come in many different forms, from pills, injections, creams and gels. Steroids are very potent and have many serious side effects. Some side effects are; impotence, infertility, heart attacks, mood swings, and an increase of bad cholesterol, liver cancer and development of the male breast.

Anabolic steroids convert into estrogen-like compounds in a process known as “aromatization,” resulting in a change of balance between estrogen and testosterone. The result of this change may be the development of breasts in men, and the testes may reduce testosterone production.

Gynecomastia, a Side Effect of Risperdal, Leads to Multiple Lawsuits

Gynecomastia is attributed to several different causes, and some remain unknown. Dr. Delgado tells his California male breast reduction patients from San Francisco, Sacramento, San Jose and beyond, that most are due to an imbalance of hormones, in particular increased levels of prolactin. Legal and illegal drugs are known to cause the development of male breasts.

In one of the country’s largest health care related settlements, Johnson and Johnson agreed to a settlement of $2.2 billion for the marketing of Risperdal “off-label.” “Off-label” use means a doctor can legally prescribe a drug for uses other than what the FDA approved it for, but the drug companies are not allowed to promote their drug for anything other than what it was approved for.

Risperdal is an anti-psychotic drug that was developed to treat schizophrenia and bipolar disorder in adults and adolescents. However, sales reps were rewarded large bonus’ for promoting the drug to pediatricians and other physicians for “off-label” use. They promoted Risperdal for; attention deficit hyperactivity disorder (ADHD), depression, sleeps difficulties and anxiety. Additionally, claims were made that j & j paid for doctors’ golf outings, and financially rewarding consulting deals.

Omnicare, which is the nation’s largest risperdallong-term care pharmacy, was implicated by a “whistle-blower,” one of their previous pharmacists, of being involved in “kickback for switches.” This is where pharmacists were “encouraged” to switch patients from approved drugs to Johnson and Johnson medications, which included Risperdal.

Some feel that the $2+ billion in fines is just a slap on the wrist. In a survey done by Stanford University and the University of Chicago, 54% of prescriptions for anti-psychotic drugs were for “off-label” uses and determined this was the result of good marketing.

Including global sales, between 2003 and 2010, Risperdal generated $24 billion in sales for johnson & johnson. Federal investigators state that j & j was aware that Risperdal could increase the risk of developing diabetes, stroke in the elderly, and development of breasts in boys, but chose to keep quiet about it.

As new evidence emerges, more individual and class actions lawsuits are being filed.
